Transferring to adult services

Transition from childhood to adulthood brings about many changes.  It is a normal “growing up” activity for everyone e.g. moving from primary to secondary school or moving up to a different age team in sport . Moving to an adult hospital is just another. However, making the change and adjusting to your new school, team, class or hospital is a different experience for each and every person. How you adjust will depend on a lot of things but particularly how well prepared everyone is. If you’ve got to know and develop trust with your “old” team moving on can be hard. Letting go and believing that the new team will do the “right thing” can be hard. Communication and trust is really important at this time. So is feeling involved in the process and having some control over when and how everything will happen.

CLDF has a series of leaflets for parents and carers to help guide your thinking and planning in preparation for your child moving to adult services. Ideally, they are best read before the move takes place so you are informed and maintain some sense of control. You can download the leaflets using the link below.
